**Nurse Practitioner / Physician Assistant - Health Risk Assessments**

**Key Responsibilities of the Nurse Practitioner / Physician Assistant - Health Risk Assessments**

- Perform thorough in-home health risk assessments for Medicare Advantage members
- Identify active and chronic medical conditions to evaluate physical, mental, and social needs during visits
- Provide education and guidance to patients regarding identified conditions and recommended screenings
- Record findings using the provided iPad
- No requirement for continuity of care, prescriptions, on-call duties, or follow-up

**Requirements for the Nurse Practitioner / Physician Assistant - Health Risk Assessments**

- Valid and unencumbered Nurse Practitioner (NP) or Physician Assistant (PA) license in any specialty except women's health or pediatrics
- Availability of at least five 7.5-hour days per MONTH
- Willingness to travel within your county and neighboring counties as needed

**Compensation**

- Competitive pay per completed visit – typically 6–8 visits daily
